They acknowledge Luffy and Zoro dealing damage, thats what you do when you want to improve yourself. Killer used his Sonic attack to do internal damage, which at least hurt Kaido, following his reaction. Exactly like Law, which was meant for internal damage, which isnt resulting in an outside scar.
