Who is the skeleton / Whats the purpose?

With all the reveals the most important and intriguing one apparently is that a death reaper (Shinigami) appears before Zoro. What are the possibilities:
1. Just a tool used by Oda to showcase Zoro's condition
2. Hawkins related stuff
3. Some side effect of Enma and Zoro calling himself King of Hell (the real Shinigami got pissed)
4. It is a real Shinigami and Zoro makes another deal with him like delayed death (borrowed time) or giving up his lifespan

Considering that Oda reportedly writes or wants to write more pages per chapter and then the editor/s force him to reduce the panels we can safely assume that this plotline is meaningful at least and might play a role later on.

This Grim Reaper seems to be related to Hawkins Tarot Card
Hawkins said: "Death Card is The Annihilation Card ... it cannot Die ... it will be Revived"
Then he said: "Show me The Result of this Battle? The Tower !"

Author explains: The Tower Card signifies "Great Change", an upheaval of the established Order, its hidden meaning indicates:
"The Opening of New Path"

In other words, all of this means:

01. Zoro represents "Annihilation"
02. He cannot Die (His Role is Unfinished)
03. He will he Revived (He will be saved from Mink's Drug)
04. His Next Battle will bring "Great Change" (Saving Wano)
05. He will Open a New Path (Use your Imagination)

It's ironic how that Chapter was thought to be about Kid vs Big Mom
But guess who saw The Grim Reaper instead? It's Zoro
Guess who is close to Dying & needs to be Revived? Zoro again

Annihilation is coming (Both in Manga & Haters Fanbase)

- In front of Zoro appears a skeleton with a large scythe (a shinigami).

So we fall into the same 3 cases:

1) It's simply Brook that Zoro mistakes for Shinigami because the medicine effect is so strong that is causing him hallucinations

2) The Shinigami is just a metaphor for Zoro's situation because he's currently struggling between life and death. So for example Shinigami will be represented like a giant shadow before Zoro, pretty much like Kaido saw 5 shadows behind Luffy in chapter 1001

3) It's an actual Shinigami

PS: if it's the 3) I'll put Shinigami on top of my Tier List, to be more specific in the God Tier which is even higher than PK Tier
