The Gorosei were surprised from an awaken DF, and the only new awaken DF in Wano are Laws and Kidds.
But let's be honest, surgery DF? The abilities of Laws DF is beyond just surgery, this is kinda the DF that name can really changed since just surgery limits its true nature.
Law can give eternity, can change souls, can control everything inside his rooms, and so many hated things, what do you need teleport in a surgery room?
Law is the first DF in the world and the real origin of all DF, the first user used the power of switching souls of all type of things in Fruits, gave it to their friends or gave his friends powers and one they died kept their souls in fruits, creating DF.
How? Well, awaken abilities is the power to do things outside your own body....

Since Law's DF doesnt have something natural, I mean, nothing from the world like Tsunamis, black holes, elements, man made things, etc.
From where is his DF from? Related to what?
