I think, he won't just make his opponents sick. There might be diseases which increase one's strength. Perhaps he puts such ilnesses on his own crewmates. Akin to the Energy Stereoids from Fishman Island.
He could also turn Lafitte female, so that Sanji won't hit him.
He could be born a girl himself, that changed his sex (not that this is likely, just an example)
He could cure himself of his own ilness. Perhaps he wasn't born with it, infects himself for training. Like Goku or Lee throwing off extra weights they trained with, to get faster in serious fight, Doc Q could make himself fully healthy.
