Kuina lost to a staircase, but fortunately we know Koushirou has got connections to the RA and we know a certain someone there that can save her life and just happened to be in the neighbourhood. He didn't know, however, about the beneficial role this concontion would have on her strength and muscle development.
The traumatic brain injury caused problems with her vision, hence the need for glasses, but her love for swords and katanas is something that could never get quenched by the trauma and muscle memory is generally not impacted by trauma, and certainly not in this case, so the skill and love for swordsmanship simply lay too deeply buried inside her subconscious; despite not knowing who she was she found an even deeper calling to the world of swordsmanship from this.
The RA thought she would be better trained by the marines as they don't have a swordsman to keep up with her passion and skill, so they decided to enroll her to the marines who immediately saw her drive and great potential and thus enlisted her as a pupil. She later advanced to become Smoker's right hand man who two years ago decided to establish himself at Logue Town. This is where she came to meet Zoro who she has got no recollection of, but the mannerisms and personality of old will still always be part of her, and Zoro picks up on that immediately and it bothers him deeply.
When you get too much testosterone the body tries to seek a balance by upping the estrogen. Hence why Tashigi got so much stronger and got bigger boobs at the same time is because she has got bitch tits, ie Gynecomastia (Gyno) which is simply an overproduction of breast tissue experienced by some body builders. Her durability is better than Zoroโs and she blocked his ass easily. Right now itโs Zoro>=Tashigi and she is keeping up
