Mihawk Canonically has both the titles.

The distinction between the two was Erkan's thing but it's clear that when Oda says Mihawk It's the greatest, he means the strongest.

What I know is this:

Mihawks title is Worlds Strongest Swordsman in the same manner that Whitebeards title is Worlds Strongest Man. This is evident by looking at how their titles are written in japanese:

Whitebeard: 世界最強の男 = Sekai Saikyō no Otoko
Mihawk: 世界最強の剣士 = Sekai Saikyō no Kenshi
