I believe that everything depends on contract:
Normal contract means that you pay some years of yours life(let's say 20 or 30) and for that you receive immortal body.
With the Holy Knights you pay even bigger amount of years to receive immortal body and new powers.
With the Gorosei you pay your whole life in exchange for body that will never age, immortality plus crazy powers.
In the last case scenario you almost in everything depend on Imu, and this is why, I believe, Gorosei became weaker when they are far away from Imu.

I think it's dumb to accuse Togashi about lying about his health, we know a lot of these mangaka especially the ones that do weekly shonen jump do go through health problems.
And I personally don't see Shueisha allowing a healthy Togashi take very long breaks.
As for why doesn't get someone else to the art, well they tried that and didn't work. Others did offer to help him by doing the art while he does the writing but Togashi refused.
Togashi does seem a bit stubborn as he really wants to do both the art and the writing.

You make it sound like he's being drafted for a war, the dude can still write and have someone else draw for him. Releasing content clearly does not matter enough to him, he wants to do things at his own pace even at the price of having nearly no content coming out, that much is clear.He wants HxH to remain a quality story, and he probably had writer's block, instead of rushing back in he wants to do things completely on his own terms. More power to him, more mangaka should do this, but they don't have the pull Togashi has either tbf.
