I don't know why Oda not only decided to make a form where Luffy's constantly laughing and hopping around like a methed up lunatic, but also chose to have most of his gags in that form happen in extremely serious situations where people that may be or actually are dying and depending on him to help them out.
Imagine being Usopp, the weakest Straw Hat, and a top tier has gripped you by the neck, and all this moron Luffy can do is pop his eyes out at the scene and go "Oh nooooo he actually tricked me!!! :000"
I mean, it's a a manga, so that's fine. It's not always going to be realistic
Even if Lucci hit a vital organ, we've seen Luffy survive a fairly big stab wound from Katakuri in WCI, so it's not that crazy.
Or if you want an example of a less important character, Hajrudin survived 80% of his skeleton shattering, even though that almost definitely popped some of his organs due to the sharp, shattered bones hitting them.

Sanji's flames didn’t hurt Robin but burned the thorns, giants wouldn't be able to do that without setting her on fire
They also wouldn't be able to cut the thorns with such precision to not hurt Robin like Zoro did
Most of them are focused on brute and devastating strength, not precision and accuracy. Most of them aren't even smart, they were known as a nation who enjoyed starting battles for no good reason, thats the opposite of smart
