While Ivankov theorized Imu had the Ope-Ope Fruit's immortality surgery, I rather doubt it. Whether it's being sustained by the Covenants or something else, whatever the nature is, Imu can only do so much, as he probably simply has lived too long.
I know he has an obvious devil theme, but if everyone in the Covenant has those hearts like Super Sommers does, I also dare say he also would have a Lich theme, too.
