All people hit with his power so far have been looking directly at him, but, we see he can control it since he used it against Marines who looked at him but not the Straw Hats and Vegapunk. Why are there sacrifices every 3 years? They call them hunting games, but really what do they accomplish, they don't need to kill everyone to take away resources, they could just conquer them or force them to pay tax / tribute.

There's a very dark explanation here, that's I'm kind of scared to get into.

Many ancient religions justified murdering children in particular, but also adults, in ritual sacrifices, in order to appease their gods.

There's a chance Imu actually feeds off of these rituals, and that the Gorosei and Celestial Dragons murdering innocents gives them power. Luffy/Nika is powered by the hopes of other people, and destroys himself to fight for them. Imu is powered by the fear/despair of other people, and destroys people to gain power.
