The First Twenty, also known as the Creators (創造主, Sōzō-shu), were the twenty Monarchs who founded the World Government. Their descendants, excluding the Nefertari Family, became the World Nobles or "Celestial Dragons".

We currently only know of

  • Nerona Family
  • Nefertari Family
  • DonQuixote Family
What are the remaining 17 families? are they even important?

Remaining possible families are
  • Figarland
  • Jaygarcia
  • Marcus
  • Topman
  • Ethanbaron
  • Shepherd
This makes it a total of 8 Families , (Though only three are confirmed , Rest are my headcanon)

  • 9 Holy Knights Families (Though it's Highly possible they under the main branch of above family not a separate Family)
  • Manmayers Family is Among this 9 Holy Knights.
which makes it 9+8 = 17 Familes.

  • Roswards Family (Main antagonist of Sabaody arc)
  • Jalmack Family? The one who visited Goa kingdom but left it so that current king can rule like Donquixote left dressrosa so that Riku family can rule
But again that makes only 19 families known so far.

1 family is still missing!!!!!!!!!! (Fishmen or giants royal family? I think that's not possible , CD seems to hate all the other species)

They are Vivi Family + Imu Family + Five Elders Families + Nine Holy Knights Families + Four Others
Aside from Nefertari Family, all of 19 Families have Godheads, basically "Leader/Head of Family"
These Heads are Imu, Gorosei & Holy Knights

Meaning there are Four more Godheads not yet Revealed
My Headcanon says that Imu & these Four were all Ruling together Secretly above Gorosei
But Imu betrayed the other Four, and hence why She alone sits on Throne

@Salah WG Word of advise. Stop fixating on the first image we got and look at new material like Kuma's flashback. There are at least a dozen royal knights. Could be thirteen, but remember Doffy's family left since.

Donquixote Family didn't leave Mary Geoise, it was only him, his brother & his Parents, not entire Family
And i'm not fixated on that Number just cuz it was what Oda revealed initially, but i simply believe Mary Geoise mirror Wano

Imo WG is basically if Wano was entire World, so instead of Flower Capital, We have Mary Geoise/Red Line & instead of Four Regions surrounding it, We have Four Blues, and instead of Fifth Lawless Region that is Kuri, We have Grand Line
(It's just that Roles & Scenarios gonna be Reversed, meaning instead of Kurozumi being Villains & Rest Good, We have Nefertari as Good Side & all Rest as Evil)

Fair, none the less, Doffy might have been predestined to fill that spot and there is no one else in the family young enough to fill in.
