We were wondering why Imu is bleeding in the lower realm. I have some thoughts about it, and I believe that love sickness might be the best explanation.

A regular devil fruit makes the user weak against the sea, whereas his devil fruit makes him suffer in the lower realm. If the user cannot effectively utilize the power, it defeats the point of making it as if the devil fruit weren't made to be used on Earth. If the devil fruit is made on the moon, then it makes sense. With the Enel connection, Moon's mural that looks like Egypt's carving, and Imu relating to Lunarian pretty much make his devil fruit an outer space creation.

The theory of Blackbeard absorbing Imu's power is very popular. If Blackbeard successfully takes the power, he will also inherit its weakness. It makes no sense for Blackbeard to fight his enemies from the cloud or from the Red Line. It's the same for Imu; I don't think Imu actually did fight Joyboy remotely. He should have clashed with Davy Jones or Joyboy in his real flesh without any weakness haunting him.

This weakness is like a deadly disease. This kind of disease is also unusual for the story. So far we haven't seen any villain who has any kind of disease up until Imu shows up. There are a few characters who suffered from deadly diseases before, such as Hiriluk, Roger, Ginny, and Kuja Empresses. Hiriluk and Roger are not curable for some reasons, and they do not have any devil fruits. But Ginny and Kuja's empresses are interesting cases that relate a lot to Imu's disease.

We have seen Saturn trying to find a solution to remove the devil fruit's weakness by experimenting on Ginny, who ends up having sapphire scale disease. Possibly Saturn is trying to find a cure for Imu, like people have been suspecting this whole time.

Imu's obsession with Lily is similar to Kuja Empress's love sickness disease. The difference with Kuja is Imu is not being able to leave the Holy Land to pursue his love. What irony. I believe the devil fruit's disease is connected to love sickness. The devil fruit will make the user's desire for love become a sickness. With how Imu has a strong desire to have Vivi, who looks exactly like Lily. The only way to cure the devil fruit's disease is likely love. Not to mention the connection between Imu being the most beautiful man and Kuja Empress being the most beautiful woman.
Interestingly, now back to what I said about Blackbeard absorbing Imu's power. The celestial dragon said people shouldn't show Davy Clan any love. And Blackbeard, who doesn't seem to love anyone, might be the perfect match with the power.

Not specifically Love Sickness, but I think He got a Twisted Version of it,
Among Zoro's Attacks, there are Three Moves that specifically represent Demons,
They are Crow Demon, Bull Demon & Horse Demon,

These imo definitely reference Three of Gorosei,
There is however another Demonic Attack which doesn't reference Gorosei, but after Imu's Introduction, I think it clearly References him,
It's called "Enbima Yonezu" which means "Charming Demon Sleepless Night" & the silhouette does feel Imu-Like.

In other words, I believe Imu represents a Charming Demon, He is basically Male & Demon Version of Boa Hancock,
I also don't think it's coincidence that Cavendish (Who also have Imu-Like Eyes) is very Charming & his entire Design revolves around that,
Mihawk too can be described as such, and when Oda drew him as a Female, He acted like Hancock, highlighting his Beauty,
(Even Sleepless Night references them too, as that's literally Cavendish's Life Story with Hakuba, and I don't think Imu Sleeps, even Mihawk's Sword is called Night too, and you can add Blackbeard too as He never Slept in his Life)

In other words, I think Imu 100% represents Embodiment of "Charming Demon",
His Curse should be Opposite of Love Sickness, meaning instead of getting Sick from being left by those you Love, Imu instead gets Sick when He leaves those who Love him (Hence why He avoids contact with everyone, because if People are Charmed by him similar to Cavendish, then He would be unable to leave them, otherwise He gets Cursed).

The only reason likes of Hakuba & Mihawk or other Saints don't have such Curse is because They aren't Rulers, as it's similar to Kuja where only Empress can have Love Sickness. (It's Price that person who sits on Empty Throne must pay, one that other Founders refused to take)

I like the way you give it a twist to to my theory which is already a bit twisted to make it more flavoured. I can see it as what Oda usually do. But there is a problem which is he didn't left Lili though, it's the other way around, Lili left him. Lili 'betrayed' him by scattering the Poneglyph and then disappeared. That's why he has a portrait of Vivi hanged in his garden and wanted Vivi who looks just like her.
