Akainu is a fullblown psychopath .

1. He of course truly lacks any remorse and empathy, just like the VA Onigumo who ordered the bombardment of a ship full of 1,000 marines, Akainu is like that, he can rationalise anything as everything goes for his mad ideology;

2. The ideology itself is the very expression of antisocial behaviour, the most adamant enforcers like Akainu and Onigumo living out their obsession with absolute justice and takes it to extreme levels of uncivilised behaviour, like their whole life comes down to a spreedsheat on how to perk up the numbers of "absolute justice" points on their ledger, where any act even at the cost of thousands of innocent lives and untold misery is a worthwhile deal for them;

3. Akainu is really concerned about his image like we saw in his discussion with the Gorosei, so he also displays narcissism;

4. He is superficially charming and manipulative, like he had no problem smoothtalking Squard into murdering his own pops

5. Akainu doesn't as of yet have any gf or female companion, even though he is the FA, which might suggest he is unable to feel love and form proper relationships, thus Akainu seems to be the guy that goes around philandering and sportfucking, breaking hearts whereever he goes. :kata:

6. Akainu's theme is also anger symbolised by the dog and of course magma, which if you combine those two traits makes for an extremely impulsive guy, as even though he is shown to be calculatingly evil and even cautious (scared of Wano ninjas), when faced with the prospect of hunting down pirates (which is his obsession) he becomes completely erratic and impulsive, like we saw in MF, even to the point where he would rather mindlessly follow that compulsion rather than stay behind and protect MF and the marines against BB that was going to sink the island.

lack of empathy, antisocial and disregarding the rights of others, narcissist, superficial charm and manipulative, unable to bond emotionally, impulsivity, yeah Akainu pretty much checks all the boxes :goyea:

When even Spandam's dad think Akainu goes completely overboard, then you know you're dealing with one mean son of a bitch

I can't tell whether Oda wrote Sakazuki's character following his concept of psychopaths because, after all, he isn't an expert; I mean, the Vinsmoke siblings were stated to be emotionless yet they very obviously showed quite a good amount of emotions.

But I'd like to comment on some of your points:

2. The ideology itself is the very expression of antisocial behaviour, the most adamant enforcers like Akainu and Onigumo living out their obsession with absolute justice and takes it to extreme levels of uncivilised behaviour, like their whole life comes down to a spreedsheat on how to perk up the numbers of "absolute justice" points on their ledger, where any act even at the cost of thousands of innocent lives and untold misery is a worthwhile deal for them;
Sakazuki's ideology isn't an expression of antisocial behaviour; in fact, it's more of an exaggerated approach to the opposite as his model of "absolute justice" actually seeks the reinforcement of civilization and social order under strict rules. If we follow Beck's schemas, those with antisocial personality disorder show self-concepts of weakness and perceived threat from others, but Sakazuki behaves as he sees himself as capable of protecting the weak, who aren't himself, from the threat of evil in the world. Let's remember that one of the nuclear beliefs of antisocials are that they must take care of themselves, but in Sakazuki's case it feels more like "I must take care of the others".

At the end of the day, absolute justice represents a very manichaean concept of assuring that law is obeyed, which goes directly against the profile of psychopaths as they show, and I quote, "failure to conform to social norms concerning lawful behaviors, such as performing acts that are grounds for arrest".

3. Akainu is really concerned about his image like we saw in his discussion with the Gorosei, so he also displays narcissism;
Just to point out quickly, this isn't necessarily narcissism.

4. He is superficially charming and manipulative, like he had no problem smoothtalking Squard into murdering his own pops
There's a common misunderstanding regarding psychopathy/antisocial personality disorder as, while it's been romanticized into geniuses capable of manipulating whoever they want, reality rarely shows this to be the case. More commonly, and while they may occasionally manipulate someone for a short time, they are individuals with immature prefrontal cortexes that causes them many of the traits that we can use to diagnose them: reckless disregard for the safety of self; consistent irresponsibility, failure to sustain consistent work behavior, or honor monetary obligations; impulsivity or failure to plan; difficult to learn from experience...

All of this actually goes against Sakazuki's profile as he's a man capable of performing medium and long-term initiatives with success, starting by his actions to reinforce the Navy and start a more direct confrontation of the emperors by moving Marineford to the New World. Psychopaths commonly are, Hannibal Lecters aside, let's say... quite dumb. And Sakazuki may be a monster, but he can function.

As I said, Oda may indeed be thinking of a psychopath when writing Sakazuki because the guy doesn't seem to have a better understanding of psychology than any other guy who isn't trained in the matter, but I don't know if I'd call him a psychopath per se.

This is a psychopath, even in piracy there is a code of honor, akainu does not, although he is a marine.
These guys who don't have sex tend to develop disorders like akainu, the lack of sex in him has already turned him into a crazy, psychopathic, mentally ill person.
He rarely smiles, I think he can't lift the stick anymore, I think his hatred for pirates was because he was captured and castrated.
KAIDO and BB on the other hand, have women and are party-goers.

He kills without giving a shet, he is deffo a psychopath.
But I have no problem with that, op needs some dark twisted mtfkers in this story just for a plus of diversity. :kayneshrug:
But knowing Oda, he is probably gonna make Akainu one of the good guys, fighting vs wg and the elders/im.
I mean look at big mom, she got 2 „good„ modes in wano, o lin and mothers mode, she is probably the next nakama, even tho she used to send heads in boxes and also killed her own kids/people, she basically cucks the whose wcl making the regulars pay with their life just for staying there.
Akainu is probably gonna have a good mode also, Lakainu-CuckKainu luffys fanboy will be a thing in future.:gokulaugh:

Well Akainu essentially represents cult of personality archetype like someone like a Stalin.

He isn't really comparable to Hitler because Hitler came up up his own vision, where as Stalin is more of following an already existing Philosophy
