When a Gorosei retires or dies, the top Holy knight takes their place.

that’s why the Gorosei have battle scars and Vary in Age. There was a Gorosei that was the same age or older than the Samurai Ghandi guy for example and he died or something and the top holy knight was selected to replace him who was the Sanji’s father guy.

(in case you don’t know, “Sanji’s father” is just a fan made name for that one youngish blond Gorosei guy)

As for how the HK can attain the rank of Gorosei, they have to be as strong as the Gorosei or well versed in politics.

That’s yet to be proven… The Gorosei when they were discussing The Nika fruit specifically say “our predecessors hid the name of the fruit”… So they acknowledge that their have been previous Gorosei before them specifically
