In the anime they tried to explain it by having her step on one of the fishes

But yeah, it's pretty weird that apparently she's the only one who fell after the ship was kicked twice

Something to add here is that Napoleon managed to stay with Big Mom throughout the fiasco despite it being a homie in the form of a hat. I was expecting it to wash away too somewhere when she fell into the sea.
