Meta reason is because of Zoro.

Oda decided to give Zoro swords that have been wielded by the two heroes of Wano, Oden and Ryuma.

He also doesn't want Zoro to simply receive a supreme sword, he wants Zoro to make his own.

So that meant that neither Ryuma nor Oden could have a supreme grade. That also means, by extension, no one else from Wano could have one because it would make them look better than Ryuma and Oden. No one in the present day story, anyway. When we get the Void Century flashback we could well see the progenitor Kozuki with one

In-series Wano probably forged the majority of meito, including the supreme blades, and they've just lost them over time to outsiders. We already know Kitetsu made one and he was from Wano.

Speaking of Kitetsu, I wonder when/how Oda is gonna bring the Nidai Kitetsu back into the story? I guess that Kaido or King/Queen could break the Sandai, and then Zoro gets the Nidai as an upgrade

A superior sword tier does not make the user stronger unless it has a special ability or it is more heavy (heavier = more AP, Zoro in Thriller Bark about Shusui). The power depends on the user entirely.
It only would give more "durability" (stronger tier = probably better material used or better forged techinque used on it, so it can resist strongest attacks on it).

So a supreme sword wouldn't make a difference for Zoro, actually the opposite. Enma with that ability forced him to improve his CoA and a supreme sword doesn't necessarily has that ability. Swords/weapons of no random people are practically indestructible in this manga anyway, and more now with the coating in Haki.

We know nothing about it, our knowledge is pretty limited.

Perhaps it is hidden somewhere, even kept as a national treasure, weird that no one mentioned it yet in that case however.

Or perhaps someone is holding a Supreme Grade and we didn't even happen to notice.

Well, Wano is closed country.
If the world knew about 12 supreme grade, then it was outside Wano. If Wano is the center of blacksmith, then most of them(maybe not all) was brought out from Wano. Speculation:shiki,shiryu; Confirmed:gandhi

But, 12 isnt gonna be 12...
If there are more high material swords, then the number will be updated. The plot hole can be maintained/explained later.
