It means that Akainu despite being bloodlusted couldn't overpower Marco. As for 'base' Akainu, what is that even supposed to mean? He has some stronger form that he wasn't using despite being bloodlusted?
Marco wasn't even using named moves at MF. Just random kicks and blocks.

This is a lot more impressive as Sabo fought the marines without help. Akainu got Curiel only when supported by half the marines. If you even look at the panel you can literally see marines in the background cheering him beating the Bobbins of the WB pirates
She said she had other things to do. She didn't have time. There is nothing to suggest he could have gotten out of it. Leverage is a thing. When you're dangling in the air it's very difficult to get any kind of force behind any strike. She could have started squeezing with both hands and while he struggled to get air, walked him to to the sea to kill him. Contrast that to Failzuki who couldn't do anything despiite being in kill mode, compared to BM's more relaxed demeanour.

Well, she is a physical monster and grabbed his neck, couldn't do anything else. Asked Perospero to finish Marco off (which wouldn't have worked anyways, since Marco is shown to take blades to the head). And Big Mom in the end decided not to "drown Marco in the Ocean" but to run away, saying she doesn't have the weapons necessary for marco.
Was Marco really helpless when having his neck grabbed? Well, Marco would've several options. He could've used his Claws to burst her away like he pushed away the entire ship:
He could also use his free hands for a blue bird attack:
We know Big Mom has a history of being tossed around and sent flying by the likes of Jinbe, Franky, Robin, Law who just used a stone. Saying Marco would be helpless is far fetched. That's why Big Mom ran instead of throwing him into the sea.
