Because he just is the underdog of the Yonkou. Shanks had the status for years, BB had it longer than Luffy, Mihawk has been a pirate for ages…Luffy just got the title, but he‘s actually not much different from Law and Kid who didn‘t get such a title. He has to prove himself first before he gets the same amount of respect. That‘s just normal.
Mihawk is not a yonko. Not is he seem by the world government or the general public as the yonko either. Heck navy soldier openly says that buggy got the title for submitting mihawk and croc under his command.
