Even if it's indeed hunger, Zoro back in the first few chapters of One Piece where he got captured by Axe Hand Morgan and he talks to Luffy, Zoro says that he endures starvation for a week, while Zoro in Wano goes off for a few days without food and he's already needed life support from Hiyori lol.

You did not specify that in the OP. Blackbeard screaming from pain and him having low endurance are two different things. Some people are more sturdy and have pain tolerance like Vergo for example. Some people have less pain tolerance but are built like tanks. Blackbeard is the latter. He took GGnM attack do the face and continued to fight like nothing as soon as his body absorbed the pain. His endurance levels are absurd.

Considering what we have seen until now if Luffy was in Zoro's place and had to take both his pain and Zoro's then there would have been 2 options imo:

1) Luffy survives extremely injured just like Zoro.

2) Luffy dies but actually unlocks his god Nika df and proceeds to kick Kuma ass before stomping Sabaody, saving Ace etc.


Aside from that another good candidate is Urouge and his pain thing.

And BB of course can deal with this without much problems if he took a gura punch from WB to the face.
