Yeah he did not recover, his body and internals and pain tolerance were just far above Beppo who was one shot by eating one fish while Zoro was eating a lot for days.

But it all stacks up, even if Zoro's crazy pain tolerance and endurance led him to just consider it a tummy ache, poison could have been easily killing him from inside.
Not sure how much of a medical science you are aware off but just because something doesn't have immediate adverse reaction on your body doesn't mean you are healed from it.

Regarding him waking up perhaps the poison wasn't deadly enough to kill him but enough to act as a catalyst for him passing out on top of all the mental stress and hunger he had
