The in character might be a problem since it limits a lot of characters that like to nerf themselves or play with the enemy. If that's the case, Sukuna might underestimate Tomura which could end up in a one shot.

That's the main issue in non bloodlusted battles, one side might allow stuff to happen that would otherwise not happen. A bloodlusted Sukuna would throw the dimensional cleave net that he
