Unfortunately, this specific market (tabletops games) is riddled by nepotism and the number of unqualified artists that get jobs increased a lot. The end result is products with really bad art or even AI art (but made by people that are unable to even write prompts properly). Most of the good artists in the genre retired and the new blood is more woke activists than artists, wasting more time being offended on twitter than working.
This is not to say that there isn't good artists but those are moving away from the hobby and getting their money elsewhere because of this nepotism.

This is a example scenario and not literal, of course. If you are using AI you should know about censorship. I frankly don't remember specific examples because I already did all the art I needed months ago and have more urgent issues in my life than playing, unfortunately.

No. They were actually way more simpler and tamer than that (although I don't doubt that X rated stuff was also blocked). Mostly refer to positions of the body or words in combination with others.
This is one image that I made at the time that was blocked a lot because of words. But I can't lie and say what I tried to write to make that. If you tried to make something like that you would get the sad dog 9/10 times.

Answering your questions: Both images are AI generated. They were chosen because they aren't actually porn but I had issues to generate them because of censorship. Mostly because of tattoos/hairstyle. But I don't remember the exact words that caused the issue.
This one is another good example. At the time, anything that specified something with the mouth (maybe because nerds tried to simulate blowjobs or things like that) would get a sad dog. Creating a woman opening her mouth like a eel took me and some people in the thread a good time. I think this issue is still in place if you wanna try.
