We dont know if hes sick and Ageless people can get sick, immortal means you live forever but you can die from sickness and stuff.

I think hes cursed to not leave MJ. He was fine in MJ, only started coughing up blood when he left it.
I'm wondering after reading the official scans, that once Imu leaves Marijoa, the Perpetual Youth Surgery ends.

This would explain why the other Gorosei were freaking out, but not Garling.

The other four Gorosei have their souls tied to Imu, and are losing their perpetual youth due to him descending to the lower world.

If this is the case, this is very interesting for Law, because it would mean that they would then need to reaquire his fruit in order to regain their perpetual youth.

Garling never got this operation in the first place, so it's in his best benefit for Imu to do this. Now, Imu and the Gorosei NEED to get Law's fruit back, and this leads Garling to also getting the perennial youth surgery.

Edit: now that I think about it, it doesn't make sense for the Surgery to be tied between Imu and the Gorosei. It would just be on Imu's end, and the Gorosei would be terrified, because the immortal force that had been guarding them would now be on a timer.

It would be very interesting if Imu had a debilitating disease before receiving the perpetual youth surgery. Losing the effects of the surgery might mean that disease would come back.

I worded it horribly.

The point I was trying to make is that I believe the Gorosei have Perpetual Youth, and Garling doesn't.

So Imu losing their perpetual youth would be good for Garling, because if he wants that surgery himself, they need Law's Fruit.

If Imu himself needs Law's Fruit, and then has the surgery performed upon him, as the new Gorosei, Garling would be next in line.
