I still believe that Lucci killed Kuina.
Lucci committed his massacre of the soldiers and pirates 17 years ago.
Kuina died 11 years ago. Lucci would have been 19, same age as Zoro when he set out.
Luffy's Fruit was stolen by Shanks 13 years ago. The WG would have been active in the east blue looking for that fruit when Kuina died.
Dragon stopped by Shimotsuki village 12 years ago.
The most wanted man in the world.
Cipher Pol was sent to follow the trail of Dragon in Goa, and followed it to Shimotsuki Village.
There, Cipher Pol finds remnants of Wano. They are ordered to kill the granddaughter of Shimotsuki Kozaburo... Kuina.
This is why Zoro and Lucci fought in Egghead. Lucci is the man who pushed Kuina down the stairs.

Personally I like the fact that Kuina died a mandane death. I feel that's one of the reasons Zoro pushes himself so hard, all to avoid that kind of non warrior death.
