Falling down the stairs is a metaphor for suicide in japan btw.Kuina killed herself.
"Fell down the stairs" is sometimes used for hiding fighting, bullying or domestic violence, but as for Kuina's case, she probably fell down the stairs literally. (Still controversial among Japanese fans, though)
Here's a quote from a sub reddit that's called 'askajapanese'. Someone asked the exact question about a person in a certain anime. In case you're more Japanese than this guy (or the others there) I'll believe you. Couldn't find a confirmation for that idiom.

I read somewhere that is was a way to say someone killed themselves.It was a japanese source too,but it was long ago.I don't a have a link to back this up.

Kuina's dad not wanting a woman let alone his own daughter surpass his and become head of the clan pushed her down the steps then stabbed her they don't have dna evidence or finger printing in the one piece world so saying she fell and landed on her sword is easy to fake

I highly doubt this tbh. Too dark and it's extremely out of the blue

That is a new plot element for Oda to write. Having a sicko/sexist person pretending to be a good guy becsuse he wanted Zoro to be his son/Dojo head eventually.

I already posted about it a while ago. There is no specific euphemism but child suicide was an epidemic in Japan in the late 90s and many families would prefer to say it was an accident of some kind than admit it's suicide
