First point is wrong in itself. It's not an illusion. Zoro actually summons or transformations into that shi. .. Second we were already this since chapter 3. " Demonic Beast ". Shi has been going on for too long.

Not every wound leave scad in this show. Luffy was stabbed by Crocodile. Katakuri. Zoro was stabbed by Kamazou. But they carry scar from Mihawk and Akainu only.
