The "payoff" was Luffy getring one-shotted by Kaido later at the start of Wano. The whole fifth emperor title was just a way of telling us he was nearing that emperor status but wasn't quite there yet, as we readers knew the end of Whole Cake was much more of a clutch escape that needed a ton...
