Bruh, a literal Shinigami appeared in the chapter. There are multiple routes that it can go, sure, but it's not "fanfic" when it literally appeared on-panel. Zoro hallucinating from the medicine is not any more valid a speculation as Zoro having a confrontation with "Death," considering that was something foreshadowed as far back as Arlong Park.

ZKK also has quite a bit of narrative build-up and significance if you actually go and read a well-constructed ZKK thread, whether it happens or not.
