It's best to assume Lucci just went under his hands and hit his stomach, doesn't make sense for a random CP0 agent to break the strongest defense while even Kizaru couldn't immediately break it with a kick (though he was obviously holding back as he didn't want to just annihilate one of his best friends like that), and Lucci << Kizaru for sure.
If it seems unlikely, just remember that we've seen Yamato do something similar in 1024 where she spun her club and hit Kaidou in on another side of his head instead of trying to overpower his defense., and something similar may have happened with Kaidou and Luffy iirc.
This panel seems that there was a shock between Lucci's attack and Sentomaru's defense. Maybe Lucci attacked twice and Sentomaru didn't saw?
And again we need to know the outcome for Kizaru's attack on Sentomaru but we already know that Kizaru sees him as a friend and warned him to prepare for the attack.

No it’s not internal damage, it’s just shigan on roids. Sentomaru‘s reaction speed is good, he noticed Lucci’s attack before Luffy could. Even with Kizaru’s warning shot, Sentomaru put his hands up before Kizaru could reach him similar to Lucci situation.
Sentomaru's reaction is not about his defense being broken. We are unclear about what really happened.
And to say that Sentomaru's reaction was better than Luffy's...
You have to understand that Luffy didn't aimed to protect Sentomaru before he got attacked by Lucci.
Luffy's speech tell us that he thinks Sentomaru couldn't see what he saw and for that he couldn't defend himself.
Sentomaru speech tell us that he thought he blocked it. So Lucci can only made an attack(or second) so fast that Sentomaru couldn't see or an internal damage. But given how Lucci's hand is with blood I bet on a second blitzed attack.
