Yes Etzel's speed reading is a bit to blame.

But, oda's art did not help. His insistence on hiding the two mothers' faces lead to this confusion. And the lil Teach panel is too focused on Teech and too zoomed in. So the prison cell bars are easy to miss.

This is not at the same level as the other incident.
