I hope Ulquorra didn't go to hell... he understood humanity in the end... I think he just simply ceased to exist since understanding humnaity/heart was against his nature/aspect of death.
Its one of the realms like Soul society, Heuco Mundo and the living world. Kubo never fully explored it but I believe that only the most evil and irredeemable people/hollows are sent to Hell.
