Killing herself makes a lot of sense tbh, but there was no reason to hide the fact. The theorists say that it was due to censorship but early OP is actually pretty brutal compared to just saying that somebody committed suicide. Shanks got his arm cut in the very first chapter, Zeff cannibalized his own leg.…

she killed herself. when kuina was written japan was going through a youth suicide epidemic and it was all over the news. the "falling down stairs is a euphemism" was always false but also, a lot of kids killed themselves, and a lot of parents lied about why.

Kuina managed to release Wado's Bankai that killed her in the process because her haki was not enough.
On a serious note, I think she might have committed suicide. Perhaps she realized she could never become the World’s Strongest Swordsman and feared that staying alive would only hold Zoro back from achieving his dream. She may have known that one day Zoro would surpass and defeat her, but more than that, she feared that if Zoro chose to stay by her side, the fire driving him to become the World’s Strongest Swordsman would fade — that their friendship would overshadow his ambition, making the dream feel less important than their bond. From her perspective, she probably felt the need to make a sacrifice to ensure Zoro stayed fully committed to his path — much like Shanks did in his own way. Given the strong Japanese themes around honor and duty, it’s possible she saw suicide, akin to seppuku, as the ultimate act of sacrifice to preserve Zoro’s dream.

I don’t think I’ve ever seen anyone have an actual source for “falling down the stairs“ being a metaphor for suicide.
Plus I’m not sure where the fuck people would look at the fight with Zoro, which has Zoro reassure Kuina that she can still reach that potential despite being a girl, and think actually, the eleven year old is going to off herself instead now.
