It's called an inference.
He saw a man named Nerona Imu in the book, heard Sabo talk about the people mentioning Imu in a present tense, and guessed that Imu is still around and got the immortality surgery (especially since someone had to get the surgery in the first place otherwise how would anyone know it exists)
