It is very complicated indeed.
As a matter of fact, Mihawk clearly cheated his way to the title of WSS.
The anime, which everyone knows is more canon than the manga, showed us that Yoru belonged to the king of baboons and Mihawk seems to have stolen it and basically painted it black which awarded him a bunch of points already.
Vista is the real WSS; he has the same skills as Mihawk (as proven by another canon material called Databooks) but with random swords.
Mihawk also paid the Lord of the coast to handicap Shanks who was the main contender for WSS (after Vista ofc).

Mihawk's title is quite clear, whoever carries a sword, Mihawk can beat him with the only exception maybe being Shanks because their fights probably ended in stalemate. When Zoro manages to beat Mihawk he is above both of them.
