I don't think that the eternal youth surgery will work without a sacrifice or it was insignifcant when it was brought up in climax of doffy's fight
An eye for an eye
But its usage being on a protagonist (luffy or any useful person for the D) as a last resort against the big bads or being used on an antagonist (probably blackbeard) as means to defy a deadly force both fit the narrative as of now

but when you think about all this arcs where law is it shows how compassionate he was with his crew, that he would do anything for them, even to others who wasnt related to his crew he showed compassion, he even told luffy that the alliance is off because he is scared that he will drag luffy and be killed by doflamingo.

what if oda was showing us this now because his destiny is fated already?
