What will happen to your stomach? It will hurt and you'll probably even cry.
You took damage and the shirt no.
Same with Sanji, but in this case, he barely got damaged. And he was impressed that the suit was not broken.

But this isn't a punch, these are perforating attacks, and those would destroy the shirt. This is Sanji taking a direct piercing attack by a huge pteranodon that intends to open a hole in his body. The suit has something to do with the guy surviving or, at least, taking virtually no damage from that.

That huge beak was big enough to not be a piercing attack, it was like a huge punch at that point.
The beak from a huge bird doesnt act like a sword, or spear, it is rly plat-wide.
