Moria robbed Ryuma's body and Shusui, so that would explain the grave's poor state... plus the Kaido years, they probably didn't have the time/resources to rebuild it, as most of Wano was battling for survival.

To be fair Kaido was destroying everything in Wano for like 20 years to feed his war efforts. All of the Ringo shrines post-Oden death look run down and tattered in Kawamatsu's flashback.

Well you have to consider the amount of time that's elapsed since Kaido's downfall.

It took a week for Luffy and Zoro to wake, but that was a chaotic time logistically so no time to do any rebuilding.
Then it took another few days for festivities + their departure.
Then it took, what, another 3 days to get to Egghead, and then 1-2 days on Egghead.
Then it took another handful of days to get to Elbaf.

So all told, you're not looking at more than a few weeks to a month in-verse time that's passed. Much of that time was probably better spent on Wano with re-allocating resources and helping the citizens find better living, rather than it was on clean up projects for the dying infrastructure.

Hell they had basically ignored the Udon prisoners and as a result Who's Who was able to build up some tiny band of misfit morons to cause trouble behind the scenes, all under the nose of Momonosuke and the Scabbards. If something of that significance could occur, then they probably haven't done enough to shape up Wano past the immediate lives of the people.

When in doubt, blame bureaucracy.

We already know Ryuma traveled outside of Wano Via the Monsters one shot, and as far as theme of opening borders that was one of the major themes of wano, but Momonosuke decided Wano wasn't ready for that yet. So Oda can still build that connction via Ryuma because Ryuma left wano before.

Yes I suspect something along those lines
