Saint is a title valid for every DC. It's like saying "marine" to Akainu or Revolutionary to Dragon; or "his highness" to some Royalty, if you want.

I feel like it’s not hidden but the average marine or pirate definitely doesn’t know this. You’d probably need to be a vice admiral minimum to get the opportunity to meet the Gorosei or go to Mary Geose and find out.
