With how One Piece is written and how characters can survive attacks or things that should be "Lethal". A single clean hit from a sword to the Neck, Heart, of Head should be an instant kill but we've seen characters survive those kinds of attacks.
The "Lethality" of an attack is solely based on the story and writing.

On topic, even though lethality is still subject to the whims of the story, there are still attacks like Akainu's that tend to be consistently lethal.
You can't really expect a lot of lethal results from a mangaka like Oda who doesn't like to kill people. You'd never see him do things like have Fuji squash people into paste and blood with gravity or have Garp's punches take off people's heads.
