BM kills people for much less, like if you refuse her invitation to her party. But the SH's wrecked her whole Castle (it was the plot box, but they thought it was the SH's doing) 2 Commanders got defeated and embarrassed her in front of the whole world.
This dialogue was before they fucked everything up. It doesn't matter who she is mad at, this whole "your family is gonna pay" card didn't played a role at all, not for Luffy, not for anybody. But it was such a huge deal back then in Zou, because you can't do anything about it.
And the reason why Oda "forgot" about it is clearly plot armor. The SH's simply can't protect people on the other side of the world, if BM wanted she could have send someone to kill their families, friends etc. It's obvious Oda wouldn't do that.
