No I really think he stretched the back of Kaido's head. The stretching line marks on his fist and Kaido's face is a good indication for it. As for this pic, it's not as clear as the latter but it could just be the same with him stretching Kaido's pupils from the inside with his arms looney toons style.

When he pulled his hand out it was black coated in haki. That white color u see is the color of kaidos head and there are stretch marks too
