They are destroying the island for 3 chapters and Pizarro didn't showed to be harmed at all.

Only when Garp directly aims at him potentially using CoC coating attacks he gets harmed. And then Coby just takes advantage that Pizarro is harmed by Garp to do a little harm himself.

Until now his DF works exactly like Pica.

It's in the chapter. Garp destroyed the skull, then Pizarro shows up all wounded. Koby destroyed the giant arm, then it literally says the damage translates to Pizarro's real arm.

It's likely it only happens with the parts of the island he's actively controlling, not with the rest of it.
