Luffy passed out (something that only happened after he beat Kaido) which proves how serious it is as opposed to the Lucci fight or the first time his G5 ran out vs Kaido

Kizaru is merely grabbing his head like Garp after being punched by Kuzan or Kaido after suffering an inferior version of that move

So the gorosei are Immortal but they're not as old as Imu. I guess they've been using the ope ope no mi throughout the years as a way to ascend to godhood in a manner of speaking.

This would explain why they're so strong. They have nigh limitless potential to get stronger since they've stopped aging and have centuries of combat experience.
