Realistically they would turn half of the women into fighters just as strong as Zoro/Sanji, and make a bunch of them have wackier designs like BM/Dadan. The Okamas would be changed to look less gross, side characters would die more often to simplify the story (not for narrative weight). The women have to low-diff opponents if they are attractive, bc many people aren't comfortable seeing a beautiful woman take multiple haymakers to the face (its part of the reason they have a mary sue problem).

Oh and the Celestial Dragons/WG would be Nazi coded to remove the slightest hint of nuance the comically evil WG has left. It'd be lame and toothless.
