Looks like Luffy is getting used to Gear 5. He used afterwards he was in his normal state and not old and exhausted.

Why would he be exhausted after using gear 5 for one attack? That’s never been the case
Let him fight for 5-10 minutes and we’ll see the wrinkly ballsack again

One logical explanation I can give is that Luffy is using G5 very often to master the form but knowing Oda it may not be even in his mind.....
