The manga has stated that Shanks rivals prime Whitebeard and Kaido, known as the strongest while the same manga shows Mihawk testing his power against Whitebeard. This clears that Mihawk has not fought top tier opponents since long time ago.

Manga shows Mihawk retrieving 3 times or escaping from Shanks and admits he doesn't want to fight him.

Manga shows that Kaido had the force to make the marines not step in his territory while fodder were after Mihawk.

What does Mihawk have? A title. A title which Shanks also has with all the other yonkos which is the top 4 strongest pirates.
