Danzou killed Shisui and prevented a peaceful agreement with the Uchihas, the fact that Hiruzen Sarutobi did nothing to punish Danzou indicates a strong negligence on his part.
Well here's the thing, it was Tobi who cast out the Uchiha, and since then they grew wary of Madara's warnings and planned a coup, it doesn't really help when an Uchiha sacks your village with 9 tails too. Hiruzen wasn't taking action, the uchiha were going to raid the village eventually and start a 4th war where god knows how many will die.
He was twisted but regardless, it was necessary action. Shisui was his only mistake, but then again this mistake was bred from countless years of hatred between the two clans, Fugaku would've done the same, unfortunately for him it was his own son.
