Looks exactly the same as in the manga though?

People complain that Kaido hasn't changed much, which is logical, since Kaido is naturally huge, bulky and has horns. But his hybrid is what was expected. He's bigger, he has sharp teeth, Dragon Horns, a tail, curved legs, Scales and claws.
The face is slightly different, in the pic I posted he has an elongated muzzle which gives him a more bestial look
The face in the manga kept more his human traits

It doesn't look exactly the same as in the manga because that fanart actually touches something that should be essential: it transforms Kaido's face. That version has dragon eyebrows and snout, and that's actually very important because the face is where the first clue to humanity lies, and by keeping most of its original traits Oda hinders the access to the "hybrid" feeling; which is why many users quickly noticed the curly eyebrows missing in the drawing of Kaido's face in last spoilers. Something similar happens to Marco, hence why many people argue he hasn't gone hybrid yet, but his hybridation feels more real because it replaces the human arms with phoenix wings.

Kaido could have changed a lot and any proper designer can see how.
