It's a combination of both. Luffy even said that AdCoA itself wasn't penetrating deep enough, right before he figured out AdCoC, and started using it.

Luffy figured out you needed to combine the two, to penetrate deep enough and cause maximum internal damage.

Zoro fainting is the norm in the manga, he did that several times but almost always got up relatively fast.
