In this scan, you can even see Kaido's new form directly above Onigashima and it is definitely comparable to the island. Also, the giant fist is in the clouds, yet it is drawn as big as the skull. Again, perspectives and angles, this fist is automatically bigger than what's drawn if we look it at that angle.
